We have an open list and welcome requests for registration from patients living in or moving to the practice area. Click here to check our catchment area.

You can register by completing a form available from reception. To save time you can also download and print a registration form. You will need to complete this by hand and bring it to the surgery.

Please complete both forms for each person registering:

Adult form , Adult Health Questionnaire

Child form , Child Health Questionnaire

Every adult registering must present themselves to reception with one form of photographic ID and one form of address verification otherwise we cannot accept any registration forms.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

Please be aware it can take up to a week to complete registration.

Please be advised that the banks surgery is a branch of the Bearwood Medical Centre. To offer you the best possible care we may require you to travel to the Bearwood Medical Practice where we have a larger range of appointments and facilities available.
